Pfizer-BioNTech Bivalent Covid-19 Vaccine
On April 18, 2023, the Food and Drug Administration amended the emergency use authorization (EUA) of Pfizer-BioNTech COVID-19 Vaccine, Bivalent to simplify the vaccination schedule for most individuals. This action includes authorizing the current bivalent vaccine to be used for all doses administered to individuals 6 months of age and older.

Check-in at the Patient Registration Desk. Vaccine will be administered in the Medical Department, Building 4.

Valid Photo ID, Insurance Card (if available) & Consent Form (link to form)
What to Expect:
Appointments are required. Check-in at the Patient Registration Desk with your Photo ID, Insurance Card (if available). Dress casual and comfortable. Masks an Temperature Check’s are required prior to entry. Please remain home if you are feeling ill. The vaccine will be administered after you complete the screening & consent form. For your safety, you will be observed for an additional 15 minutes in the waiting area. Vaccination Cards will be provided to you.
Ample parking is available in the main parking lot. Please have your documents ready when you check-in. If you are unable to print and pre-fill the consent form prior to your arrival, printed copies will be provided.
