The medical department provides primary care, internal, pediatric and family services to the community, along with enabling services such as case management. The approach of the medical department is to offer integrative care, realizing that wellness depends on healthy mind, body, and spirit, as well as good community health. As a result, the medical department coordinates with all other providers, dental, behavioral health, traditional, and alternative, to deliver optimal, coordinated, affordable, accessible, culturally appropriate care for the people of Moloka’i. We created a health home for the people of Moloka’i, and welcome you and your family to our practice.
At the center, you will have a provider who will be your primary health care provider. She or he is your first contact with any health problems or concerns, and is responsible for coordinating all your care, both at the center and with outside referrals.
- Preventive health/annual physical exam – screening for disease, and learning how to prevent it, and stay healthy
- Well baby visits
- Lactation counseling – having problems with breastfeeding
- Adolescent care
- Acute care visits – when you are sick
- Chronic disease visits – work with our team of providers, including medical, behavioral health and dental, as well as traditional and alternative practitioners to manage your diabetes, high blood pressure, high cholesterol, and asthma
- Wart removal
- Skin biopsies
- Women’s health (pelvic exams and clinical breast exams)
Please contact the Medical staff and request your refill when you have about one week of medication remaining. Have your prescription bottle in hand to be sure all information can be relayed. Kindly allow up to 48 hours for this process to be complete. Pain medications will require a monthly appointment with your provider to refill, so please do not forget to schedule your next appointment at least 2 days before your medication will run out.

The patient-centered medical home (PCMH), is a team based health care delivery model that provide comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes. PCMH is “an approach to providing comprehensive primary care for children, youth and adults.”
