Molokai Community Health Center is a place of healing and wellness. Our medical department offers integrative care, merging wellness in mind, body and spirit, using western, traditional, and alternative treatment. Our goal is to ensure that you and your family receive the best quality care through exceptional medical expertise and by putting your safety first.
At the center, you will have a provider who will be your primary health care provider. She or he is your first contact with any health problems or concerns, and is responsible for coordinating all your care, both at the center and with outside referrals.

Contact the Medical department to request a refill on your medication. Before calling, please be sure you have at least 2 days of medication left, as the refill process may take up to 48 hours to complete. Pain medication refills require a monthly appointment with your provider before a refill can be completed. Be sure to schedule your next appointment at least 2 days prior to your medication running out.
The Primary Care Medical Home provides health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans.
